Title : Clinical effect of low-energy double-pass 1450 nm laser treatment for acne in Asians

Objective: Most of the research for the 1450nm diode laser was conducted on Caucasian and people of African descent. This research is to prove that the 1450nm diode laser treatment is effective for inflammatory acne for people with Asian skin tones.

Background data: Existing research shows that high energy (12-14J) and long cooling periods (50ms) increase the frequency of PIH, which is one of the side effects of laser treatments. Such side effects have been observed in approximately 40% of the cases. [1] In the case of acne treatment using the 1450nm diode laser, the typical dosage was 12J – 14J. The disadvantages of this treatment are pain and PIH. Therefore, the energy range of 8J – 12J may be more effective for acne treatment. [2,3] 

Methods: In this research, patients received low energy 1450nm diode laser treatments on five to ten occasions, within a timespan of 2-4 weeks. There were 30 patients in total, and 3 were excluded for health and other reasons. The data for 27 patients was evaluated according to the ranking scale of their acne. In this research, the group of patients consisted of Fitzpatrick skin types III–IV (mostly Asians).

Operating Condition: The Smoothbeam device was used (A device with the same mechanism as Hironic A-FIT™)
Spot size - 6mm
Fluence - 8 - 12 J/cm2 
Pulse duration - 210 ms 
Dynamic cooling device spray duration - 20ms
Repetition rate - 1 Hz. 
Anesthesia – Local anesthetic cream, containing 7% lidocaine (one hour prior to treatment)

Results: The evaluation of the treatment was made one month later. 78% of the subjects showed a reduction in acne, and the acne grade was reduced by 63% on average. 25 patients could endure the pain, and 2 patients asked for anesthesia. Furthermore, there were no cases of PIH, due to low energy and short cooling time duration.

Conclusion: A low energy 1450nm diode laser treatment can be a solution to reducing pain and PIH for the patient.
